  1. * T0 for patients randomised to group 1, T1 for all patients but only if they revisit their GP
  2. # T1 is the time point to measure a follow-up visit by the patient 4 weeks after inclusion. If a patient does not return to the GP, a questionnaire is sent by mail after 6 weeks. This time point is indicated as T1.5.
