  1. PF: Practice Facilitator (practice-level intervention); PP: Prevention Practitioner (patient-level intervention).
  2. PF/PP: Combined practice-level and patient-level intervention.
  3. CAD: Canadian dollars.
  4. Body mass index (BMI) is the weight in kilograms divided by the square of the height in meters. Obesity is defined as a BMI ≥30.
  5. GAD-7: Generalized Anxiety Disorder. Score for the 7 items ranges from 0 to 21; scores of 5, 10 and 15 represent cut points for mild, moderate and severe anxiety, respectively [37].
  6. PHQ-9: Patient Health Questionnaire-9. Scores for the nine items ranges from 0 to 27. Scores of 5, 15, and 20 represent cut points for mild, moderate and severe major depressive disorder, respectively [38].
  7. MOS Social Support Questionnaire: Scores for the 22 items range from 0 to 100, higher scores indicate higher level of self-perceived support [39].
